Letrozole is a potent, non-steroidal aromatase inhibitor (AI) that selectively blocks the aromatase enzyme responsible for the peripheral conversion of androgens (androstenedione and testosterone) into estrogens (estrone and estradiol). Indications

Ovulation Induction in Women with Anovulatory Infertility (e.g., PCOS)
Controlled Ovarian Hyperstimulation in ART protocols

Clinical & Safety Profile

Contraindications:

● Hypersensitivity to Letrozole or excipients.
● Premenopausal women with intact ovarian function (unless under specialist infertility protocol).
● Pregnancy and lactation (Category X).
● Severe hepatic impairment without specialist supervision.

Potential Drug Interactions:

● May interact with tamoxifen or estrogen-containing therapies, reducing efficacy.
● Concomitant CYP2A6 and CYP2C19 substrates may require dose adjustment.
● Alcohol use may exacerbate CNS-related adverse effects.

Adverse Reactions:

● Common: Hot flashes, fatigue, dizziness, arthralgia, headache, sweating.
● Oncology patients: Osteoporosis, bone fractures, cardiovascular events (long-term use).
● Infertility patients: Multiple pregnancies, ovarian cyst formation, mild ovarian hyperstimulation.
● Other: Nausea, edema, mood changes.

1. Bone Health: Long-term use may cause bone mineral density reduction; monitor and consider calcium/vitamin D supplementation.
2. Cardiovascular Risk: Use cautiously in patients with history of ischemic heart disease or dyslipidemia.
3. CNS Effects: May cause dizziness or fatigue; caution with driving or operating machinery.
4. Fertility Use: Strict monitoring with ultrasound and hormone testing required during ovulation induction cycles.
